A 53 year old male presents corneal opacity after a mycotic keratitis in left eye. We attempted DALK and a cataract surgery. During the surgery after the stromal debulking the deep stroma was opaque but we perform a phacoemulsification to prevent complications presented on open sky, and complete with a penetrant keratoplasty using intraoperative OCT. The surgery was successful and his BCVA 20/60.
